چکیده

Methods Healthy volunteers (n=70; 35 male; 35 female; median age 46 years) and patients with systemic AL Amyloidosis (n=56; 36 male; 20 female; median age 62 years), with clinical indications for CMR scanning, additionally underwent multi-organ ECV measurement. Technical details were: gadoteric acid, 0.1mmol/Kg plus infusion, multibreathhold T1 measurement and equilibrium imaging of heart, liver spleen and biceps muscle at 1.5T (Siemens Avanto). Amyloidosis patients also underwent serum amyloid P component (SAP) scintigraphy to score liver and spleen involvement by amyloid.

Quantification of myocardial extracellular volume fraction in systemic AL amyloidosis: an equilibrium contrast cardiovascular magnetic resonance study.

BACKGROUND Cardiac involvement predicts outcome in systemic AL amyloidosis and influences therapeutic options. Current methods of cardiac assessment do not quantify myocardial amyloid burden. We used equilibrium contrast cardiovascular magnetic resonance (EQ-CMR) to quantify the cardiac interstitial compartment, measured as myocardial extracellular volume (ECV) fraction, hypothesizing it would ...

Cardiac amyloid burden assessment by T1 mapping predicts survival in patients with systemic AL amyloidosis - a 2 year follow-up study

Background Cardiac involvement drives outcome in systemic AL amyloidosis. Late gadolinium enhancement (LGE) CMR is useful for the detection of cardiac amyloid, but characteristic LGE patterns do not always occur or may appear late in the disease. Using CMR T1 mapping, we measured the pre contrast myocardial T1 and myocardial Extracellular Volume (ECV), reflecting myocardial amyloidosis burden a...

T1 mapping and survival in systemic light-chain amyloidosis

AIMS To assess the prognostic value of myocardial pre-contrast T1 and extracellular volume (ECV) in systemic amyloid light-chain (AL) amyloidosis using cardiovascular magnetic resonance (CMR) T1 mapping. METHODS AND RESULTS One hundred patients underwent CMR and T1 mapping pre- and post-contrast. Myocardial ECV was calculated at contrast equilibrium (ECV(i)) and 15 min post-bolus (ECVb). Fift...

3.0T Cardiac Magnetic Resonance Quantification of Myocardial Extracellular Volume using different delay time of post-contrast T1 mapping for the Diagnosis of Cardiac Amyloidosis

Background Cardiac involvement in systemic light-chain (AL) amyloidosis is caused by the extracellular deposition of misfolded AL immunoglobulin and carries a poor prognosis. Cardiac magnetic resonance (CMR) T1 mapping and extracellular volume (ECV) measurements may have advantages over late gadolinium enhancement (LGE) for early diagnosis and quantifying of amyloid burden. The aim of this stud...

Extracellular volume fraction mapping in the myocardium, part 2: initial clinical experience

BACKGROUND Diffuse myocardial fibrosis, and to a lesser extent global myocardial edema, are important processes in heart disease which are difficult to assess or quantify with cardiovascular magnetic resonance (CMR) using conventional late gadolinium enhancement (LGE) or T1-mapping. Measurement of the myocardial extracellular volume fraction (ECV) circumvents factors that confound T1-weighted i...
